Registrants > Registrant > Follow-up Questionnaire Page

The Registrants > Registrant > Follow-up Questionnaire page displays the follow-up questions that have been added to the Registrant's record. Users can view Registrant information, the Registrant's questionnaire, and their responses to the follow-up questions as follows:

Note: -_This page is only available if the Registrant has been sent follow-up questions.

ClosedView Follow-up Questions

Click the checkboxes that correspond to the follow-up question groups that you want to view. The question group section expands to display the answers provided to the questions.

ClosedView All Registrant Information

  1. In the Registrant Information section, click the View All Registrant Information link.

    The Registrants > Registrant Information page for the Registrant displays.

  2. To view the Registrant's contact information, select the View Contact History checkbox.

    The Registrant's contact information displays at the bottom of the page.

ClosedView Questionnaire PDF

Click the Questionnaire PDF button to view all Questionnaire questions and responses for the Registrant in PDF format.

A new tab opens in your Web browser containing a PDF with the Registrant’s responses to the questions in addition to their name, SSN, EDIPI number, Service Status, Registry Status, and the dates on which they started and completed the Questionnaire.

Note: -_Help Desk users cannot view the Questionnaire PDF, so the Questionnaire PDF button is not displayed.
